This came my way today, from Medscape*:

Does the time of day of immunotherapy infusions affect patients’ survival outcomes?

A recent meta-analysis that examined data from over a dozen studies concluded that immunotherapy provided earlier in the day substantially improves progression-free and overall survival across a range of cancers, including metastatic melanoma, renal cell carcinoma, and esophageal cancer, as well as both locally advanced and metastatic non–small cell lung cancer (NSCLC).

In stage IV melanoma, for instance, a 2023 study found that patients who had more than 75% of infusions after 2 PM had shorter median overall survival — 14.9 months vs 38.1 months for those with earlier infusions (hazard ratio [HR], 0.45).

@Dan_M (and anyone else), if you have any means to influence time of therapy then maybe worth using this as guidance

Not sure about others but mine normally starts before 9am and finishes around 2pm. It can take up to (and more) 8 hours depending on the individual, first time I had it I had a reaction (not uncommon apparently) it took almost 12 hours and I had to be omitted to hospital as the day treatment centre closes at 5pm.

Interesting. Must take a look at at that.

Pretty surprised they actually had data relating to the timing of the therapy, superficially it seems quite odd. Did they suggest a reason for it such as diurnal variations of body immune responses/cortisol release etc?

There would clearly be practicalities involved if the timing is eventually proven important.

Numbers are not huge (73 I think) in the linked analysis of a melanoma study though it’s fantastic researchers are looking at so many ‘out of the box’ factors. The abstract mentions this is not something adopted in clinical practice currently but very interesting nonetheless.

Although the impact of circadian timing on immunotherapy has yet to be integrated into clinical practice, chronoimmunotherapy is an emerging and promising field as circadian oscillations are observed in immune cell numbers as well as the expression of immunotherapy targets

How are things Dan? Cold symptoms any better?

Been ill with some sort of virus. Cold and cough, temperature for a day, followed by diarrhoea for 24 hours. Haven’t eaten anything since Sunday. Just fluids.

Blood tests were fine strangely. Treatment 12pm today. Apparently its only an hour.

Feel exhausted. Hoping to eat soon!

My son and daughter were both fine before the weekend. They both got ill after me. So I have made them ill.

If my kids are to my knowledge ill I don’t see them. If they are well then I do see them.

I have to see them especially the 4 year old regularly. It’s bad enough being ill and not working. Not seeing my kids would be a step too far.

I have to live a life of some kind.

I agree, and not seeing them isn’t an option but you do need to be cautious.

Just had a call from hospital. Treatment being delayed as it is known to cause diarrhoea. They want me fit before I start. Bloods friday, treatment monday.

Probably for the best as I have only just started eating again. Feel very weak.

A shame but as you say, it’s probably for the best. Whilst waiting for the start of treatment, I hope you can get out and enjoy the lovely weather we are having at the moment.
